For some, the foundational functional medicine model is different from prior experiences, so we want to give you a clear picutre of how root cause healing is different from band-aiding symptoms with synthetic medications.

During your discovery call, or when you registered online, you received access to our Blue Pebble Health Patient Portal. This is a secure way for you to complete your intake forms, upload documents and labs, track your symptoms monthly with the MSQ form, and ask us questions.

Please complete the intake forms in your Blue Pebble online portal. Please note all your concerns, past medical history, imaging and any prior lab testing. We like to review all your forms and uploads before your appointment. We kindly ask you to complete the intake forms with a red start button next to it, at least 24 hours before your appointment to maximize your time with your provider. We recommend completing the intake forms on a computer instead of your phone so you can see the whole form with ease and upload documents.We love to know about so many areas of your health that some of our intake questionnaires are long, so please set aside sufficient time to complete them. If you start a form and can't finish it, please click the "Save and Continue Later" button to save what you have completed thus far. Once you hit "Submit", you cannot make changes but will have to start another form. Please upload any old labs or imaging reports that you think are helpful. 1. Get to know YOU and discuss your concerns and review your medical history. We want to know - What are your top concerns and symptoms?What trends have you noticed in those symptoms? How is this effecting your quality of life? What foods are you concerned about? Do you have any studies (like xrays, CT, MRI) or prior labs to review? Do you have any pain? What essential oils, herbs or supplements are you currently using? Are you taking any medications? Would you like to get off of them? Do you believe you can fully heal? If you felt better what would you be doing?

2. Create a comprehensive lab order to understand your baseline biomarkers and health status looking at your blood counts, liver and kidney functions, electrolytes, inflammatory markers, food issues, comprehensive hormone panel, including thyroid panel, vitamin levels, mineral levels, other personalized tests based on your history. We have many options for testing and often you can use your medical insurance to cover the labs. Let us know if you need to use a certain lab or need your labs coded "wellness" for better coverage.

3. Its better to test and not guess! Go for your lab testing at the location best for you. (Most often we use labcorp, but your lab order can be taken anywhere- please let us know of your preference) We also have special cash pricing on our comprehensive lab panels. Go to lab in the morning before 9am, ideally after an overnight fast- no food after 8pm, but do drink purified water to optimize hydration in preparation for your lab draw. We do adjust the amount of blood drawn based on age and size.

4. Your labs are back in about 2 weeks! Hooray, you are ready to discuss your results at a Report of Findings consult! We will look at optimal ranges, NOT lab ranges. We will take the findings as pieces of a larger whole, telling a bigger story of where things are and what you need.

If you need to cancel or reschedule your follow up appointment, please allow us 24 hours notice to avoid a $50 cancellation fee. Call or text 801-797-0877 to cancel or reschedule. If you have a request for something requiring your providers' time outside of your appointment time, you might be charged for that time. These convenience fees might apply for example to, doctors notes, forms to be signed, insurance letters, prescription refills etc - please bring those to your appointment so they can be done during your appointment time to avoid any extra fees. Forms fees are $30-$50 if done outside of appointment times. Refill fees (when requested outside on an appt time) are $50 if you have been seen in the last 6 months. If its been more than 6 months, please make an appointment to discuss the refill. Portal messages are for simple "yes or no" questions between visits. If your question requires more detail, or you have several questions, or you have not been seen in the last 3 months, please make an appointment, or ask about our membership options so we can serve you the best!

There are several tested- high grade brands we love. Most can be found on your personal link to Fullscript, which you should receive an updated email after your provider visit if your supplements have changed. We can also help you check out from your fullscript cart while you are at the office for an easy " in-office" check out, as some items we will have in stock in Lehi. Our Lux Blanc brand in our in-house brand, and we can ship those items from our office to you. Those items can also be purchased online at our store. In general, We recommend getting enough to last you 3 months, so you can complete your program before the next testing is completed.

Since we spend so much time with you, we can not use the traditional insurance billing methods that are based on how many prescriptions you need. Unfortunately, it is a broken system of sick care, not of restoration, wellness or preventative care. We can provide you with a 'super bill' that allows you to submit your receipts for reimbursement depending on the type of plan you have you might receive reimbursement for all, part or nothing. Investing in your health now, will save a lot of time, money and heartache in the long run and allow you a longer and more valuable life- where your lifespan and healthspan are equal.

As a Specialized Functional Medicine practice we do not accept insurance or bill insurance companies. We recommend calling your insurance company prior to your visit to find out what they will cover.

Yes we do! Many people find telehealth to be a convenient way to receive the health and wellness care they need while remaining in the comfort of their own home. We can offer coaching services to clients in all states. Our providers offer telehealth appointments to those who live in PA and UT, and we hope to add other states soon. Should you live outside of these two states, and want to see our providers instead of one of our coaches, you can come in person to your first appointment to the UT office and follow up can be over telehealth during the remainder of the year.

You can always find your key documents such as invoices, lab results and wellness plan or supplement list in your Patient Portal. Generally your labs results are available in the portal AFTER your visit is completed. Our office provides hard copies of these forms at each visit and they can be emailed after virtual appointments, upon request. If you need us to send forms to other providers or create duplicates, the cost will depend on the time required as per our Convenience Fees.

If you have a tumor, cancer, terminal or chronic illness- yes working on your foundational wellness, nutrition and inflammation is a huge part of the healing path. As Functional Medicine Specialists we do not handle emergency care and we do not regularly check messages after hours or on weekends. However, if it is during our open hours and you have a mild illness or cold, fever, sore throat or mild injury - like a twisted ankle yes please give us a call and we can get you set up with natural wellness options for faster recovery.

Your Discovery call with Caroline, our new patient coordinator, is complimentary to help you figure out if we are a good fit for you and if you are a good fit for our programs. If you are a good fit, then we schedule an initial appointment with one of our providers. The initial fees run from $397-$597 depending on the provider. There is a refundable $200 deposit collected at the time of scheduling and the remainder of the visit fee is collected after the completed appointment. Most of our patients opt to use insurance for most of the lab testing. Some lab testing is out of pocket costs, but those tests are optional. The Report of Findings appoitment, when you review your labs with your coach or provider is the same cost as the initial visit, as their is a larger investment of time to prepare your personal plan. Generally follow up visits are every 3-6 weeks in the first few months and can range from $99-$329 depending on the time and provider. Our plan is to repeat your labs at the 3 month mark and adjust your plan depending upon how fast your symptoms and labs are changing. We recommend planning on having your labs checked 3-4 times in the first year for best results. Each time we check fewer labs as more of them come into the optimal ranges and will not need to be checked as often. We routinely see autoimmune markers go negative in the first 3-6 months, then we like to check them yearly to be sure they remain negative. We are working on creating more bundled visits and services to save you more. We find that when you commit to the recommended follow up plan, you get better results faster as we are able to make small adjustments more often. We find those with the slowest recovery path, are those who call us only when things are going poorly for their health. We will also recommend some personalized nutritional supplementation and possibly other wellness services if you need them, such as IV therapy or Red Light Therapy. We do offer package discounts on most of these services. We will soon introduce our community membership plan that has build in discounts.

We spend extensive time with you on your wellness plan. Insurance companies do not value this time, which is why we do not bill them for our services. We provide you with an invoice for you to submit to the out of network portion of your insurance for possible reimbursement. There also may be times when healthcare costs might count as a tax deduction.

We accept cash, personal check, all credit cards, debit cards, HSA/FSA cards. We can also accommodate payment plans, when discussed before the visit or service date. Note: If you have a phone or virtual telehealth appointment, we require a card on file as we will process your payment directly after your appointment. You can also log into the portal right after your appointment and make a payment directly. We appreciate your prompt payment!

Opened bottles or purchases made over 14 days ago will not be considered. If you accidentally received the wrong supplement and its within 1 week of purchase, yes. Other circumstances are taken case by case basis and there is a restocking fee.

Knowing your personal biomarkers, investigating your triggers of inflammation and health stressors is an essential part of getting to the root cause of your concerns. You will speak with your provider and decide what set of labs will provide the best insight to your health situation. Lab costs vary, depending on what labs you need to investigate your specific health needs. Our typical panels cover more than 50+ tests- vitamins, minerals, inflammation, hormones, foods, organ function, electrolytes and blood counts. However, most specialty functional medicine testing, like the Dutch test, GI MAP, Vibrant's TotalTox panel are not covered by insurance, and generally those are optional as we try to use these only when needed.We choose the highest quality labs and provide you with cost-effective self-pay options. A typical cash pay panel ranges from initial testing panel $300-$800 and follow up panel $75-$350. Our office does not directly bill insurance for lab work, but the lab can bill your insurance in most cases.​ If you have insurance coverage for labs, we will send you to a lab such as LabCorp and they can bill your insurance. To understand whether your insurance company will cover lab work, we recommend calling your carrier directly. They can help you determine your coverage, the amount of your deductible, whether your plan covers labs before or after your deductible is met, whether you have a co-insurance or a co-pay and what that exact amount is, and what lab is included in your plan. Most of our patients do use their insurance for our comprehensive bloodwork panels and do not pay a lot in out-of-pocket costs, but each plan in different.
