Frequently Asked Questions

How do I login to my BPH Patient Portal?

https://bluepebble.md-hq.com/

For quick access, you can also use the blue “Patient Portal” button at the top right of the menu bar on our website.

If you're having trouble logging in, feel free to text us—we’ll be happy to resend your patient portal welcome email. Please note that the link in the email is only valid for 24 hours, so be sure to use it promptly.

What are the steps to becoming a new patient?

We invite you to schedule a complimentary 15-minute discovery call with Caroline, our New Patient Coordinator. This call is a great opportunity for you to see if our practice aligns with your health goals—and for us to determine if you’re a good candidate for the specialized care we offer.

You can schedule an appointment at your convenience by clinking the following link:

https://bluepebble.md-hq.com/embedded/schedule.php

Before your call, we recommend watching our short video, “What is Functional Medicine?” It will help you understand our approach and come prepared with any questions you may have. Many find that functional medicine’s focus on root-cause healing is quite different from conventional care, which often centers on managing symptoms with synthetic medications. We want to provide you with a clear picture of how our approach aligns with your journey toward lasting wellness.

How do I prepare for my initial consult?

When you scheduled during your discovery call or registered online, you received access to our Blue Pebble Health Patient Portal and should have received a welcome email to prompt you to create a password. This secure portal allows you to:

-Complete your intake forms

-Upload documents and lab results

-Review forms, labs, receipts, handouts and other documents

-Track your symptoms monthly using the MSQ form

-Communicate directly with us through the Secure Messaging

What We Need from You Before Your Appointment:

Please log in to your Blue Pebble Health Portal and complete the required intake forms. These forms have a red START icon next to them.

We ask that you:

-Complete your forms at least 24 hours before your appointment to maximize your time with your provider

-Include details about your concerns, medical history, past imaging, and lab work.

-Upload any previous labs or imaging that may be helpful

-Please upload a photo of yourself for identification verification

-Save a preferred card to process virtual appointments and online purchase requests

A Few Tips:

-We recommend using a computer instead of a phone for the best experience and to easily upload documents

-Some forms are detailed (we love to understand your whole health picture!), so please allow yourself enough time to complete them

-If you need to pause, use the "Save and Continue Later" button

-Once you hit "Submit", edits are not possible—you would need to start a new form if changes are needed

We look forward to seeing you in your personal consultation soon!

What is your philosophy?

At Blue Pebble Health, we believe in the power of the body to heal when given the right support. As a naturally minded integrative and functional medicine practice, we focus on foundational wellness principles to restore your body’s God-given ability to thrive.

How Did We Get Here?

It’s not your fault. Modern life has placed unprecedented stress on our health—fast-paced routines, nutrient-depleted diets, chronic stress and anxiety, lack of movement, poor sleep, and constant toxic exposures have left even “healthy” people struggling. But here’s the good news: It’s not too late.

Knowledge is power!

In nearly every new patient evaluation, we identify 3-5 key nutritional deficiencies—often overlooked yet crucial for optimal wellness. Through advanced testing, we pinpoint the most important stressors affecting your body and create a personalized, simple-to-implement plan that delivers massive results. Small, targeted changes can make a profound difference—we routinely see autoimmune markers resolve within 12 months or sooner!

A New Perspective on Your Health

The missing piece to your health puzzle is different from what you’ve been told before. It’s time to take a fresh approach—one that actually works.

Our Money-Back Guarantee

We are so confident in our approach that we guarantee you’ll gain a NEW, personalized insight into your root cause and the best path to restoring your health—or your money back.

What would I expect in your program?

At Blue Pebble Health, we specialize in helping you uncover the root causes of your health challenges and unlock real, lasting solutions on your wellness journey. We recommend starting with a comprehensive foundational Functional Medicine deep dive, beginning with an initial visit with one of our experienced providers.

Step 1: Get to Know YOU

Your initial consultation is all about you—your health story, concerns, and goals.
We'll explore:

-Your top symptoms and concerns

-Patterns and trends you’ve noticed

-How your health is impacting your quality of life

-Any foods you’re concerned about

-Whether you’ve had prior labs, imaging (X-rays, CT, MRI), or other diagnostics

-Any pain or discomfort you’re experiencing

-Your current use of supplements, herbs, essential oils, or medications

-Whether you're interested in reducing or eliminating medications

-Your belief in your ability to fully heal

And a key question: If you felt better, what would you be doing differently in your life?

Step 2: Comprehensive Lab Testing

Its better to test and not guess! Based on your history and symptoms, we’ll create a personalized lab panel to evaluate your baseline health—including:

-Blood counts

-Liver & kidney function

-Electrolytes

-Inflammatory markers

-Food sensitivities or issues

-A comprehensive hormone panel (including thyroid)

-Vitamin & mineral levels

-And other specialty tests, as needed

We offer flexible lab options:

-Most testing is done through LabCorp, but you're free to use any lab you prefer—just let us know

-Insurance may cover much of your lab work

-We also offer special cash pricing for comprehensive panels

Lab Prep Tips:

-Schedule your lab draw before 9 AM, ideally after an overnight fast (no food after 8 PM)

-Drink plenty of purified water to optimize hydration

-We adjust blood draw volume based on age and body size for your comfort and safety

Step 3: Report of Findings – Your Personalized Health Roadmap

About two weeks after your lab draw, it’s time for your Report of Findings! We’ll go beyond “normal” lab ranges and look at what’s optimal for you. Think of your results as puzzle pieces—we’ll help you see the full picture of what’s really going on and what your body needs to feel its best.

This is a powerful and eye-opening conversation, where we’ll:

-Gain Clarity

-Explore your results using optimal ranges (not just conventional “normal” ranges). We’ll interpret your biomarkers as clues to your body’s deeper story.

-Personalized Natural Solutions: Receive a tailored roadmap based on your results—this may include repleting nutrition through supplementation and nutritional upgrades, and other lifestyle strategies to restore balance and health.

-Empowered Action: Take steps toward lasting, vibrant health—knowing that we’re here to guide and support you every step of the way!

What are your cancellation fees or convenience fees ?

If you need to cancel or reschedule your follow-up appointment, please give us at least 24 hours' notice to avoid a $50 cancellation fee.

Call or text us at 801-797-0877 to make changes.

Additional Time & Convenience Fees

Requests that require your provider’s time outside of your scheduled appointment may incur additional fees. To avoid this, please bring any of the following to your appointment:

- Forms, doctor’s notes, insurance letters: $30–$50 if completed outside of a visit

- Lab changes or additional requests to an existing order

- Prescription refills: $50 if requested outside of an appointment and you’ve been seen within the last 6 months.

* If it’s been more than 6 months, please schedule a visit to discuss your medication.

Portal Message Guidelines

Portal messages are for simple "yes or no" questions between visits, unless directed otherwise. If your message is complex, involves multiple questions, or if you haven’t been seen in the past 3 months, please schedule an appointment or ask us about our membership options for ongoing support.

How do I refill my Supplements?

FULLSCRIPT:

We work with several trusted, high-grade supplement brands that we love. Most of these can be found through your personal Fullscript link, which you’ll receive via an updated email after your provider visit if your supplements have changed.

If you can't find an item listed in the recommendations from your provider in your Fullscript account, please contact the office and we will assist in getting those items to you.

For your convenience, we can assist you with checking out from your Fullscript cart while you’re at the office or by phone or text, making it an easy “in-office” purchase. We also keep some items in stock at our Lehi location.

OFFICE EXCLUSIVE & SPECIAL ORDER ITEMS:

Our Lux Blanc brand is our exclusive in-house line, which we can ship directly from our office to you. These items are also available for purchase online through our store.

- Cellcore, Intramax, Zinc and a handful of other items need to be shipped from our office. You can use the following order form for those or call the office for assistance while we work on updating our online store! Ordering Form

We generally recommend getting enough supplements to last 3 months, so you can complete your program before your next round of testing.

Do you take insurance?

At Blue Pebble Health, we spend dedicated time with each patient to understand the full picture of your health. Because of this, we can not participate in traditional insurance billing, which is typically based on quick visits and the number of prescriptions written—a model focused more on managing illness than restoration, wellness or preventative care.

We are a Specialized Functional Medicine practice, which means we do not bill insurance directly. However, we are happy to provide a superbill after your visit. You can submit this to your insurance provider for possible reimbursement. Depending on your plan, you may receive full, partial, or no reimbursement.

We highly recommend contacting your insurance company before your visit to understand your out-of-network benefits. Here’s what to tell them:

- You are seeing an Out-of-Network Provider at Blue Pebble Health

- The billing code for your Initial Comprehensive Visit is 99205

- The billing code for your Follow-Up Visit is 99215

Investing in your health today can save time, money, and heartache in the future—allowing you live a more valuable, longer, healthier life where your lifespan and health span align.

Do you offer telehealth, virtual or phone visits?

Yes, we do offer telehealth!

Many people find virtual visits to be a convenient and effective way to receive high-quality care from the comfort of their own home.

We offer coaching services to clients in all states, and our licensed providers currently offer telehealth appointments to residents of Pennsylvania (PA) and Utah (UT). We’re actively working on expanding to more states soon!

If you live outside of PA or UT and would prefer to work with one of our providers (instead of a coach), you're welcome to travel to our Utah office for your initial visit. After that, your follow-up appointments can be done via telehealth for the rest of the year.

Where can I find my labs and treatment plan?

You can always access your key documents—including invoices, lab results, wellness plans, and supplement lists—in your Patient Portal!

Please note that lab results are typically uploaded to the portal after your visit is completed. At in-person visits, we provide hard copies of your materials, and for virtual appointments, you can find them in your portal.

If you need us to send documents to another provider or create duplicates, convenience fees may apply based on the time required. You can view more about these fees in our Convenience Fee Policy.

Can Functional Medicine help with serious conditions?

If you're facing a tumor, cancer, chronic, or terminal illness, know that there is always something that can be done to support your body. At Blue Pebble Health, we believe that foundational wellness—nutrition, inflammation reduction, stress support, and lifestyle balance—can play a powerful role in your healing journey, no matter where you're starting from.

We don’t see your diagnosis as a limitation, but as a starting point to uncover what your body truly needs. Functional medicine helps us ask deeper questions, identify root imbalances, and create a plan to strengthen your resilience and improve your quality of life.

There is no concern too big or too small. We’re here to help you discover the meaningful “pebbles” of health wisdom that guide and empower you.

Healing doesn't always mean curing—but it always means supporting the body, mind, and spirit in the most intentional way possible.

What regenerative and restorative procedures do you offer?

We offer many options for those wanting natural joint recovery (Prolozone, PRP, Ozone, VSELs, cellular products), anti-aging supports (like NAD, Red Light and other injections), Metabolism booster - fat melter and weight loss program, Song Laser, brain enhancement, pelvic floor support and various nutrition, peptides and restorative injections. If you are ready for more advanced support, please speak with your provider.

Do you have urgent or emergent appointments?

In case of an emergency, go to your local hospital emergency room or dial 911. As Functional Medicine Specialists we do not handle emergency care and we do not regularly check messages after hours or on weekends. However, if it is during our open hours and you have a mild illness or cold, fever, sore throat or mild injury - like a twisted ankle yes please give us a call and we can get you set up with natural wellness options for faster recovery.

What is a typical investment?

Your Discovery Call: The First Step

Your complimentary Discovery Call with Caroline, our New Patient Coordinator, is designed to help determine if we’re the right fit for your needs—and if our care model aligns with your health goals.

If we decide to move forward together, we’ll schedule your Initial Consultation with one of our providers.

Initial Visit Fees

Initial Consultation Cost: $397–$597, depending on the provider

Deposit: A refundable $200 deposit is collected at the time of scheduling

Final Payment: The remaining balance is collected after your appointment

Lab Testing

Most patients choose to use insurance for standard lab testing. Some specialty labs may involve out-of-pocket costs, but these are optional and always discussed with you first.

Report of Findings Appointment

About two weeks after your labs are drawn, you’ll return for your Report of Findings appointment. This is a powerful and informative session where we connect the dots between your symptoms and lab findings—and map out your next steps for healing and progress.

-This visit is the same cost as your initial consultation ($397-$597), as it includes in-depth review and preparation of:

- Your lab results, analyzed using optimal—not just standard—ranges

- Your personalized wellness plan

- Supplement, nutrition, and lifestyle recommendations tailored to your needs

Follow-Up Visits & Lab Monitoring

We find that when you commit to the recommended follow up plan, you get better results faster as we are able to make small adjustments more often. Those who only reach out when their health declines tend to progress more slowly.

-Follow-Up Visits: Cost: $99–$329, depending on the time and provider.

Typically every 3–6 weeks in the first few months, as needed if critical, and follows the lab ongoing lab monitoring as outlined below.

-Ongoing Lab Monitoring:

- 3 months- Labs are usually repeated at the 3-month mark to adjust your plan depending upon how fast your symptoms and labs are changing

- 3-4 times the first year- We recommend planning on having your labs checked three to four times in the first year for best results. Each time we check fewer labs as more of them come into the optimal ranges and will not need to be checked as often.

- Yearly- We routinely see autoimmune markers go negative in the first 3-6 months, then we like to check them yearly to be sure they remain negative.

Additional Therapies & Discounts

Your provider may recommend supportive services such as:

-IV Therapy
-Red Light Therapy
-Personalized Supplements
-Softwave Therapy

and others as your circumstances may benefit! We offer package discounts on most services and are soon launching a Community Membership Plan that includes built-in savings.

Insurance & Reimbursement

Please refer to the question “Do you take insurance?”

What are my payment options?

We accept the following forms of payment:

- Cash

- Personal check

- All major credit and debit cards

- HSA/FSA cards

We’re also happy to accommodate payment plans when arranged prior to your visit or service date.

For Phone & Telehealth Appointments

- A card on file is required for all virtual appointments.

- We will process your payment immediately after your visit using the card on file as payment is due at the time of service, unless previous arrangements have been made.

- You may also log into your Patient Portal and make your payment directly following your appointment.

Thank you for your prompt payment and for valuing the time and care we provide!

Can I return a supplement?

Supplement Return Policy:

- Opened bottles or purchases made over 14 days ago are not eligible for return.

- If you received the wrong supplement by mistake, you may return it unopened within 7 days of purchase.

- Other return requests are considered on a case-by-case basis and may be subject to a restocking fee.

What is a typical labwork panel?

Understanding Your Biomarkers and Lab Testing

Knowing your personal biomarkers, investigating your triggers of inflammation and health stressors is an essential part of getting to the root cause of your concerns.

During your visit, you’ll work with your provider to decide what set of labs will provide the best insight into your unique health situation.

Lab Testing Overview

- Our typical panels include 50+ tests, covering vitamins, minerals, inflammation markers, hormones, food sensitivities, organ function, electrolytes, and blood counts.
- Specialty functional medicine tests (like Dutch Test, GI MAP, Vibrant TotalTox panel) are usually not covered by insurance and are optional, used only when necessary.
- We prioritize high-quality labs and offer cost-effective self-pay options.

Cost & Insurance

- Cash-pay prices for initial panels typically range from $300–$800, with follow-up panels between $75–$350.

- Our office does not bill insurance directly for labs, but most labs (e.g., LabCorp) can bill your insurance directly.
- To understand your coverage, we recommend calling your insurance carrier to confirm:
- Lab coverage details
- Deductibles and copays/co-insurance amounts
- Which labs and providers are included in your plan

Most of our patients use insurance for lab work and experience minimal out-of-pocket costs, but coverage varies by plan. We’ll guide you through the process to make testing as affordable and effective as possible.

Our Money-Back Guarantee

We are so confident in our approach that we guarantee you’ll gain a NEW, personalized insight into your root cause and the best path to restoring your health—or your money back. (Just bring us the doctors notes within 1 week of your ROF, that show you were told the same thing in the past.)

How others are achieving wellness!

For the past eight years, I struggled with relentless fatigue and saw over 15 different doctors— lots of basic testing, tons of prescriptions, but none of whom could give me clear answers. Then I found Dr. Herzog. From our very first appointment, I felt heard. Unlike my previous experiences, Dr. Herzog took the time to listen and ordered comprehensive testing that no other doctor had considered. The results finally gave me answers—my body was battling chronic immune stress, which was draining my energy. Now, just four weeks into my personalized program, my energy has skyrocketed from a 3/10 to an 8/10! I feel like a whole new person, and for the first time in years, I’m excited for what’s ahead!

- HA 48y

With my background as a health coach, I’ve always sought the root cause of health issues and prioritized natural solutions—for myself and my family. Finding an office like Blue Pebble Health has been life-changing. They not only share my approach but also provide the tools and guidance that truly work for me. One of my biggest struggles was fertility. After years of frustration and unanswered questions, Blue Pebble Health helped me uncover underlying imbalances that were impacting my ability to conceive. Through their thorough testing and personalized support, I finally had a clear path forward. Today, I am beyond grateful to say that I’m experiencing the joy of motherhood again—something I once feared might never happen. It’s rare to find providers who are both willing and knowledgeable enough to think outside the box. Thank you, Blue Pebble Health, for your dedication and expertise! - MW 37y
